Rehabilitation Services
Each of our residents comes to us with very different and specific rehab needs. Our entire rehabilitation team is committed to creating individualized therapy programs that directly answer those specific needs. We encourage each resident to achieve their maximum potential of independence and ability.
Post-Acute Care
At GEM Transitional Care Center we seek to maximize the quality of life for our post-acute care population, who require a specialized restorative program. The therapy team and nursing staff will develop individualized treatment plans for resident care.
Palliative Care
Palliative Care optimizes quality of life by anticipating, preventing, and treating suffering throughout chronic and life-threatening illness. It addresses the resident’s physical, intellectual, emotional, social, and spiritual needs.
Activities and Dining Program
A positive and uplifting atmosphere is vital to the success of healing, recovery, and rehabilitation. GEM Transitional Care Center employs outgoing and friendly professional staff to create social programs and activities to fit the residents’ needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Friends and family are always welcome to visit and attend our special events.GEM residents enjoy appetizing, nutritious meals, which are prepared under the supervision of a Registered Dietitian. Those residents not on a therapeutic diet can choose from a delicious range of dishes that rotate daily or they can order from the Always Available menu.
Social Services
Social services provides essential information, manages requests and concerns, coordinates appointments, maintains contact and open communication between the facility, families, and the residents, and helps in care planning and discharge planning for each resident.
