Social Services

The Social Services Department assists residents in adjusting to our center, along with identifying their psychosocial needs, and assists with their discharge planning. Social Services is the liaison between GEM Transitional Care Center and the families. The staff is available to assist with any individual counseling needs, transportation to appointments, and the coordination of resources such as:

• Dental Care
• Vision Care
• Podiatry Care
• Audiology Care
• Psychosocial and Psychiatric Evaluations as needed
• Individual Therapy
• Advance Directives
• Aftercare resources
